Do you take my insurance?
Headquarters Physical Therapy is a cash-based, out-of-network PT practice that aims to provide the highest quality 1-on-1 care without the restrictions of third-party payers. This model only benefits YOU.
You can send in a self-claim to your insurance with a claim form provided on your insurances website along with the necessary info from a superbill (provided by us). The amount of reimbursement or payment toward your deductible is dependent upon your insurance plan. You should contact your insurance provider prior to your first appointment and inquire about your out-of-network benefits and be sure there is no other requirements needed to get started.
How much do our services cost?

Our services are usually reimbursible by your insurance company at about 40-80% of the total cost. Please contact your insurance company directly to inquire about your out of network benefits.
Do I need a physician referral?
In short, no...you do not need a physician referral to utilize our physical therapy services.
Do you only treat high level athletes?
No, we have a significant amount of experience in the wellness management and treatment of elite athletes, however, we see just as many patients who come in with chronic and long-term issues. We treat musculoskeletal injuries of all types. We can treat anything from neck pain to ankle and foot issues.
What can I expect at my first appointment?

This includes a thorough evaluation with a highly qualified Doctor of Physical Therapy who is dedicated to serving the patient with the intention of reaching goals and education on specific diagnoses, as well as, general wellness. What do I need for my first visit?
Wear loose fitting clothing that allows you to move. Arriving 10-15 minutes prior to your scheduled visit time will ensure that you get the most out of your session.
